The Pennsylvania School Health Law requires physical exams for students entering the 6th grade. Please submit your child’s physical exam and any updated immunizations to the school nurse by 9/7/22.
The physical exam must be dated on or after 7/1/21.
All Required Paperwork for HEALTH and SCHOOL are due before the First Day of School:
